New/incoming student-athletesThe University of Nevada requires all freshmen and transfer student-athletes to complete a pre-participation history and physical examination prior to participating in practice or competition. Every athlete must have their history and physical examination on file in the Athletic Training room. This history and physical examination is very comprehensive and will identify existing medical problems which might impair athletic performance/physical health. Please fill out the following MANDATORY forms prior to your pre-participation examination with the sports medicine staff:
Finally, the University of Nevada requires that ALL students (athletes and non-athletes) have their immunization records on file at Admissions and Records. Students are not allowed to register for classes until Admissions and Records receives this information. Upon successful completion of the pre-participation exam the student athlete will be given clearance to practice and participate in all sport related activity including practices, competitions and strength and conditioning sessions. Student-athletes will not be permitted to participate if the pre-participation physical is not completed. Student-athletes will be notified by medical personnel if the pre-participation examination is not successfully completed. Pre-existing injuries and illnesses are not covered by the University of Nevada. Medical testing and procedures for conditions discovered during the Pre-Participation Exam may be covered by the University. Returning Athlete Physical UpdatesAll returning student-athletes are required to complete a Medical History Update Form prior to the beginning of their respective season. Failure to complete this form and consult with a member of the Athletic Training/physician staff will result in athlete being ineligible to participate in his/her sport.
Walk-On Student-Athlete Physical ExamsWalk-on student-athletes are required to complete a pre-season physical exam form, medical history questionnaire and have a physical exam performed by a physician at the Nevada Student Health Center. The walk-on student athlete is responsible for the cost of this exam. In addition, the Nevada medical history questionnaire must be completed along with a parent insurance information form. All costs associated with any tests or medical procedures necessary to complete medical certification will be the responsibility of the student/parent.
